Clinical Services and Supports

We offer a variety of case management services to meet your needs. Case Management is a personalized service that creates a well-developed care plan, based on assessments. That plan is developed in consultation with you and reflects your choices and preferences for service. The goal is to ensure that you are involved in all aspects of the planning, service arrangements, and delivery.

Services:

• Brief consultation (up to 30 minutes): This is a phone call or meeting to get quick answers to questions and community resource lists. (Free)

• Case management in-depth consultation: This is a meeting to explore specific concerns, options, and available resources. Support, education, and guidance in dealing with a client’s concerns are provided. The consultation is also available by phone for families who reside out of town. ($50)

• Full case management assessment (up to four hours): This is a home visit to fully assess a person’s current situation, including physical, cognitive, emotional, social, and financial needs, as well as available supports, the safety of the living environment, and quality of life. Options are explored and a plan with recommendations and referrals to community resources is developed to address the client’s needs and concerns. ($200)

• Follow-up case management services (up to one hour): Follow-up case management services are only available to clients who have already received a full case management assessment. This is suggested if a client’s situation and needs have changed, requiring a reassessment of needs and changes to the previously developed plan. In some instances, follow-ups may be longer and incur additional charges if the client needs additional help exploring their options and added guidance, support, and education to make decisions. ($50)

Jewish for Good provides these services on a sliding scale fee to those who qualify. The fee is based on gross annual income and the size of the household. Client fees may be re-evaluated periodically to reflect changing circumstances.

Our Emergency Financial Assistance Program provides families assistance for necessities such as housing, utilities, health care, and food in times of crisis when they are unable to provide for themselves. Emergency Financial Assistance may be provided when there are medical issues, a loss of income, or a death in the family; however, it is not intended to provide full financial support for any individual on a long-term basis. In cases of ongoing need, our staff will help the individual or family to find other resources. Anyone receiving financial assistance will need to meet with one of our clinical social workers for an initial interview and undergo a financial assessment. All applicants will be asked to provide documentation related to their income and their expenses.

Emergency Financial Assistance is considered on a quarterly basis on the following dates January 3rd, April 1st, July 1st, October 1st. To be considered for assistance, you will need to complete THIS FORM between 7am and noon on those dates. People will be selected at random after 12pm.  If you are selected, you will be contacted within three business days from the first of the quarter to schedule a meeting for a financial assessment.

The Food Pantry enables us to serve the community by providing non-perishable food and toiletries to those in need.  Additionally, in collaboration with Farmer Foodshare, we are able to offer fresh produce to those experiencing food insecurity. 

General Food Pantry pick-ups occur on the 4th Tuesday of the month, from 1-3pm by appointment only. RSVP required.

Jewish mourning customs reflect the natural course of grief and recovery following the death of a loved one. We are committed to assisting those in need navigating this challenging time.

We partner with a local grief coalition to provide a weekly support group.
